Health Plans

With more health conditions being tracked and accommodations made for students to attend regular school regardless of the medical condition, school personnel that interact with a student need to be made aware of the condition, how to recognize warning signs, and how to react in critical conditions. Using the Health Plans tab on the student Health page, you can select a health plan template and customize it for the student based on their specific needs for the condition.  Once a health plan is associated to a student, an alert appears on the student's record indicating to other users that interact with the student. Once a user has read and understood the health plan for the student, a read receipt is logged for that user. 


Work with Health Plans

Note: Drop-down lists are populated using the Health Code Sets

Note: One or more health plans may be attached to a student, but only one per health condition.

  1. On the start page, search for and select a student.
  2. Under Information, choose Health from the student pages menu. The Health page appears for that student.
  3. Click the Health Plans tab. The Health Plans page appears.

To view all health plans:

Select the Show All(+0) checkbox to view all health plans (health plans that have yet to start, active health plans, and expired health plans). 
The count (+0) indicates the total number of expired health plans for the selected student. Expired health plans will appear in italics.

To create a health plan:

  1. Click Add. The Create Health Plan drawer appears.
  2. Do one of the following:
    • To create a health plan using a pre-defined template, choose the appropriate template from the Health Plan Template drop-down list.
    • To create a health plan from scratch, leave the Health Plan Template drop-down list blank.
  3. Enter information as needed:

    Description

    Name

    Enter a name for the health plan.

    Description

    Enter a description of the health plan.

    Start Date

    Enter the date the health plan becomes active. Defaults to today's date.

    End Date

    Enter the date the health plan expires, if applicable.

    Status

    Choose the status of the health plan:

    • Draft - A preliminary version of the health plan
    • Completed - The health plan is published
    • Ended - The health plan is no longer applicable

    Note: A nightly job updates the status of any expired health plans to Ended.

    Details

    Use the rich-text editor to quickly and easily enter and format the content you want to appear within the health plan.

    Note: If using a health plan template that contains smart tags, the smart tags will automatically be translated into student and related data. Any tag data that is not available or set for the student is marked as --.

  4. Click Submit. Health plans that are active (valid Start Date and Completed status) are visible within the Medical Alert pop-up.

To edit a health plan:

  1. If needed, use Basic Filter to narrow the list.
  2. If needed, sort by column heading
  3. Click the name of the health plan you want to edit. The Edit drawer appears.
  4. Edit information as needed.
  5. Click Submit.

To delete a health plan:

  1. If needed, use Basic Filter to narrow the list.
  2. If needed, sort by column heading
  3. Click the Delete (minus sign) button next to the health plan you want to delete. A confirmation message appears.
  4. Click Delete.

To view change history details for Health Plans page:

If Change History is enabled, the Change History link appears at the top of the page. Click the link to view the Change History Details for that page.

To print a health plan:

  1. If needed, use Basic Filter to narrow the list.
  2. If needed, sort by column heading
  3. Click the name of the health plan you want to edit. The Edit Health Plan drawer appears.
  4. Click Print.

View Medical Alert 

If enhanced health is enabled, the pop-up displays the student's active health concerns, active health plans, and a status indicating if a plan has been read and acknowledged by the user.

Note: If enhanced health is enabled, once the student has at least one active health concern, legacy active medical alerts display along with the active health concern. Additionally, active health plans associated to the student will also display.

  1. Click the Medical Alert icon. The Medical Alert pop-up appears.
  2. Click View Plans. The Health Plans page for the student appears.
  3. Click the name of the health plan. The View/Acknowledge Health Plan drawer opens.
  4. Once you have read through and understand the health plan, select the I acknowledge that I have read and understand this health plan checkbox.
  5. Click Acknowledge.

Note: The plan continues to display in the alert even after you have read and acknowledged the health plan. The status of the plan in the alert reflects whether it requires acknowledgement. If a health plan is updated, the status of the plan in the alert will indicate the need to re-acknowledge.

Work with Read Receipts

To add a read receipt:

  1. Click View for the health plan for which you want to add a read receipt.
  2. Click Add.
  3. Enter the Name (Last, First) of the person who read the health plan.
  4. Enter the Date on which this person read and acknowledged the health plan.
  5. Click Save.

To view a receipt:

  1. Click View for the health plan for which you want to view read receipts. 
  2. Click Close when done viewing.

To view all read receipts:

  1. Click Read Receipts.
  2. Select the Show All(+0) checkbox to view read receipts for all health plans (active health plans and ended health plans). The count (+0) indicates the total number of read receipts for Ended health plans for the student. Read receipts for Ended health plans appear in italics.
